  • ____  What were the dates of the candidate s employment?
  • ____  Was the candidate a responsible and trustworthy employee?
  • ____  Did the candidate interact positively with other employees?
  • ____  Did you receive any complaints regarding the candidate s work performance or conduct? If so, specify the circumstances.
  • ____  Based on your knowledge of the candidate s qualifications and skills, do you believe the candidate will effectively perform the duties required in this new position?
