We are investigating a specific incident or activity which resulted in economic loss or injury to the company. You had access to the property that is the subject of the investigation and we have a reasonable suspicion that you were involved in the incident or activity under investigation.

The specific incident or activity being investigated involves:

The basis for our reasonable suspicion that you were involved in the incident or activity under investigation is as follows:

(The basis for reasonable suspicion calls for evidence that is independent of the polygraph, and presumably exists before the polygraph is administered, which leads the employer to suspect this particular employee. Only with this additional supporting evidence can you lawfully administer a polygraph as part of an ongoing investigation.)

The foregoing serves as our basis for asking you to submit to a polygraph test.
