Sample Training Evaluation Form


Note Before designing a form, review the objectives of the training program. Design the form to gather data in order to measure whether or not the training program met its objectives. The danger in using generic forms, such as the one reproduced below, is that they are not designed to measure specific objectives. However, the sample form is helpful in ensuring that the instructor and the facilities are addressed in the evaluation.

Identifying information. The form should have identifying information, such as:

  • Course title
  • Name of company
  • Course number
  • Instructor name
  • Date(s) of training
  • Site/location
[You may request the name of the participant; however, generally the form is anonymous to encourage candor.]

Directions: Respond to each statement below by checking the item that most clearly indicates your opinion on the following scale:
