OSHA Form 300A: Annual Summary of Work-Related Injuries and Illnesses
Form 300A, Summary of Work-Related Injuries and Illnesses must be posted at each establishment in the place or places where notices to employees are customarily posted. Posting on a total company basis, rather than on an establishment-by-establishment basis, is not permitted. The summary must be posted from February 1 to April 30.
The Annual Summary is a compilation of injuries and illnesses recorded on the OSHA 300 log during the previous calendar year. To prepare the Annual Summary, employers must review the OSHA 300 log entries to verify that they are complete and accurate. A company executive must certify on OSHA Form 300A that he or she has examined the OSHA 300 log and that he or she reasonably believes that the Annual Summary is correct and complete. The company executive who certifies the Annual Summary must be one of the following persons:
- An owner of the company (only if the company is a sole proprietorship or partnership) ;
- An officer of the corporation;
- The highest ranking company official working at the establishment; or
- The immediate supervisor of the highest ranking company official working at the establishment.
In instances where there are no injuries or illnesses during the year, zeros must be entered on the totals line and the form must be posted.
The regulations require that posting of the prior year's injuries and illnesses take place no later than February 1 every year and that the form remain in place until April 30. Summaries need not be posted for those establishments of multi-establishment employers that have been closed down during the calendar year.
